Simply visit our website and click any of the 'Get Started' or 'Make Appointment' buttons. You'll fill out a secure application form — make sure all information is accurate, as it is entered directly into the Florida State Registry. Once submitted, you'll receive an email to schedule your appointment at a date and time that works for you. At your appointment you'll sign consent forms, meet with an educator, and then see your physician.

After completing your online application, you'll receive a link to schedule your appointment. We offer in-person evaluations at our Oakland Park and Jacksonville locations. New patients must visit in person for their first evaluation to establish a patient-doctor relationship. Renewals may be available via telehealth.

While it is strongly preferred to have a pre-existing diagnosis, it is not legally required.
